The Belleville and Quinte West Community Health Centre, on behalf of the Quinte Health Link, is seeking applications for a part-time (.8 FTE)

Project Manager, Quinte Health Link

Positions Summary:
The Project Manager, Quinte Health Link is responsible for leading the Health Links initiative in Quinte region with an overall goal improve client outcomes, quality of care and reduce costs for clients dealing with complex health and social needs.

Core Responsibilities:

  • Establish and maintain appropriate structures and processes (ie. steering committee, working groups, etc) to engage local stakeholders – providers and clients/community - in Health Links 
  • Work with the local stakeholders to ensure understanding of the Health Links and how it aligns with related strategies such as sub-regions and the like
  • Identify, plan, coordinate and evaluate HL initiatives using a quality improvement approach
  • Work with organizations to increase and expand local adoption of a HL approach and initiatives including coordinated care planning 
  • Coordinate with local stakeholders to identify and work to overcome system barriers to care for people with complex care needs
  • Manage central intake of referrals and assign accordingly.
  • Ensure Quinte Health Link is aligned with, and leverages, LHIN-wide and provincial HL initiatives as appropriate.
  • Prepare and manage HL budget
  • Prepare, implement and evaluate HL work plan, and ensure it is aligned with sub-regional and other appropriate system priorities
  • Recruit, orient, train, supervise, coach, Health Link staff

Qualifications:
The successful candidate will have: a Master’s degree in a relevant discipline or equivalent; five or more years of progressive management experience; a strong understanding of health equity; demonstrated expertise in change management, quality improvement, program planning and evaluation; strong leadership skills; and excellent communication skills.
