Caregiver Capacity Building Project Coordinator
1 Part-Time (12 Month Purchase of Service Agreement)

Rexdale Community Health Centre is a multi-service Centre providing primary health care, settlement and community development activities for women, families, youth, adults, and seniors of diverse cultures. The Centre is committed to providing accessible and sustainable programs and services to diverse communities.

The Centre is currently seeking a Part-Time (contract) Project Coordinator to join our team.

The Project Coordinator will oversee the development, implementation and evaluation of the Caregiver Capacity Building project. The Rexdale Community Health Centre Caregiver Capacity Building Project will focus on four distinct areas: skills training & knowledge building; self-care for caregivers; social supports; and, expanding the catalogue of caregiver resources – print and online, and in multiple languages. The project will incorporate a number of delivery methods, including: hands-on practical workshops; information presentations from program area experts and professionals; peer support group sessions; individual counselling; and, multi-format, multi-lingual resources.

The successful candidate will work closely with the Seniors Program Manager, Seniors Program Team, volunteer community members, partner agencies and organizations to deliver this exciting project.

Deliverables:

  • Develop and lead a project advisory (Steering Committee) made up of key stakeholders and community partners to identify needs, priorities and strategies for effective project delivery 
  • Design and Develop a stakeholder Intake tool as an access point for program and systems support navigation 
  • Develop a Caregiver resource packet 
  • Develop a multilingual on line resource kit 
  • Supervise project staff

Qualifications and Assets:

  • Undergraduate degree in a health sciences or other relevant discipline or equivalent work experience
  • At least 3 years work working with individuals of diverse cultures in a community based setting
  • Demonstrated skills in community development and project coordination
  • Must have great leadership/initiative skills
  • Demonstrated administration skills
  • Strong oral and written communication skills
  • Creative problem solver
  • Ability to work flexible hours, including evenings and weekends
  • Ability to speak a second language is an asses (i.e., Punjabi, Tamil, Twi, Hindi, Spanish, Somali)
  • Knowledge of the Rexdale community is an asset


Salary Range: $29.46 per hour
Hours per week: Up to 17.5 hours per week (flexible as per needs of the project)
Starting date: June 2018
Status: Non-unionized position
