Community-governed Family Health Teams

Ontario’s Community-governed Family Health Teams (CFHTs) provide primary care for their communities but are different than other provider-led FHTs. CFHTs are governed by a community board of members from the local community.   Because of the focus and belief in community outreach, CFHTs do all that they can to reach out to meet the needs of their community, going outside the boundaries of primary clinical care.
