October 20, 2012 to August 5, 2013
|Key Areas of Responsibility:
Clinical Services: assessment, diagnoses, treatment, chronic disease prvention and mangement, health education and counselling, working to the full scope of RN(EC)
Collaboration/consultation with a family physician as required
Continuity of Care includes home visits as requred
Workig in a manner that incoporates a health promotion approach to the determinants of health.
To achieve quality improvement, uses quality assurance practices consistent with evidence-based best practice guidelines.
Collaboration with other members of our interdisciplinary team including family physicians, dietitian and community health promoters.
Baccalaureate or Masters in Nursing with successful completion of post graduate Primary Care Nurse Practitioner Program
CNBO Extended Class License, in good standing (or licensure in province current residence and eligibility for CNO RN (EC)
Current membership with RNAO, including professional liability insurance
Primary health care experience, preferably in community setting
Demonstrated commitment to interdisciplinary team environment
Experience with electronic health records an asset
Experience in program development, implementation and evaluation an asset
Valid Driver's license, an asset
