Practising at Carlington Community Health Centre (CCHC) provides longer appointment times and lower patient volumes than many other primary care settings. We have four friendly and supportive physicians who work closely with a multidisciplinary medical team of nurse practitioners, RNs, chiropodists, dietitians, seniors outreach workers, and medical secretaries. Patients are also supported by cross-team work with counselors, social workers, and other health and social services professionals. We offer an opportunity to work specified hours and practice in a manner that treats the whole person in the context of their individual circumstances.

Working within the context of community health principles and practices, DECNET provides diabetes education support and services to adults with pre-diabetes and Type 2diabetes in East Toronto. Particular emphasis is placed on increasing access for mental health, hard to reach, complex, and marginalized populations and members of our community.
